- To apply for Parking Accommodations, students should complete the Parking Accommodation Forms which can be found on the Student Affairs page of the portal.
- The student must complete the Request for Parking Accommodations.
- The treating physician must complete the Physician’s Verification Form.
- Both forms should be returned to the Student Affairs Office via email [email protected] or returned to room 202 of the 1928 building.
- The Associate Director for Academic and Student Programs will process the request and will notify the student of a decision via email.
- The Administrative Services Office will be notified and will reach out to the student with the
corresponding parking pass.
