Academic

1. To apply for Academic Accommodations, students must complete the Academic Accommodations Request Form found on the Student Affairs Page of the Portal.

2. The request form must be completed by the student as well as the treating physician.

  • Student must complete Section A.
  • Treating physician must complete Section B and provide supporting documentation for Section C (requests will not be processed without both B and C)

3. Once complete, forms should be returned to the Academic Affairs Office for processing, room 203 of the 1928 building or emailed to [email protected].

4. It is important to submit the request as early as possible to give the Academic Accommodations Committee ample time to review the request prior to exams (at least two weeks recommended).

5. Once a decision is made, the student will receive an email letter from the Associate Dean of Academic Affairs.

6. The Registrar’s Office will be notified and will reach out directly to the student prior to exams with exam details.
